Initial Comments:
An offsite follow-up survey completed September 1, 2022 found that Extended Family Care had corrected the deficiencies cited under the requirements of 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of a federal recertification survey completed on June 29, 2022.

Code, Part IV, Health Facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of an onsite state relicensure survey completed June 29, 2022.
